Assumption of Risk:
I understand that participation in activities at Little Masterpieces, including but not limited to painting, crafting, and activities involving food, carries inherent risks. These risks may include, but are not limited to:
Allergic reactions: Exposure to various food items, paints, glues, and other materials may cause allergic reactions. Even if precautions are taken. While we take precautions , the pre-packaged food products we use may have come into contact with allergens.
Skin irritation: Contact with certain materials may cause skin irritation.
Minor injuries: Slips, falls, or accidental contact with art supplies.
Ingestion of non-food items: Despite supervision, there is a risk of accidental ingestion of paint, glue, or craft materials.
Culinary art / workshops:
I, the undersigned parent or legal guardian of the participant, acknowledge that participation in a culinary workshop involves potential risks, including but not limited to: use of sharp utensils, hot surfaces, electrical appliances, allergic reactions, and potential food borne illness. I understand and accept these risks on behalf of my child.
I acknowledge that it is my responsibility to inform Little Masterpieces of ALL my child's allergies, dietary restrictions, and any other relevant health conditions. I understand that Raana Mateen and the Little Masterpieces team will take reasonable precautions to minimize risks and accommodate known allergies to the best of their ability. However, I understand that it is impossible to guarantee a completely allergen-free environment.
Release of Liability:
In consideration for my child being permitted to participate in activities at Little Masterpieces, I, on behalf of myself, my child, our heirs, executors, administrators, and assigns, hereby fully and forever release, waive, and discharge Raana Mateen, the Little Masterpieces team, its employees, volunteers, and any associated individuals or entities (collectively referred to as "Little Masterpieces") from any and all claims, demands, actions, causes of action, damages, losses, expenses, and liabilities of any kind or nature whatsoever, whether known or unknown, foreseen or unforeseen, arising out of or in any way connected with my child's participation in activities at Little Masterpieces, including but not limited to any illness, injury, allergic reaction, or other adverse event, even if such illness, injury, allergic reaction, or other adverse event is caused by the negligence (but not gross negligence or willful misconduct) of Little Masterpieces.
Indemnification:
I agree to indemnify, defend, and hold harmless Little Masterpieces from and against any and all claims, demands, actions, causes of action, damages, losses, expenses, and liabilities (including reasonable attorneys' fees) arising out of or in any way connected with my child's participation in activities at Little Masterpieces, including but not limited to any claims arising from my failure to fully disclose my child's allergies or health conditions.
Emergency Medical Treatment:
I hereby authorize Little Masterpieces to seek and consent to emergency medical treatment for my child if, in the reasonable judgment of Little Masterpieces personnel, such treatment is necessary. I understand that Little Masterpieces will make reasonable attempts to contact me in the event of a medical emergency. I agree to be responsible for all costs associated with such emergency medical treatment.
Behavior & Dismissal:
I understand that respectful behavior is expected. Disruptive behavior may result in dismissal without a refund at the discretion of the instructor.
Photography/Videography Release :
I grant Little Masterpieces permission to take photographs and/or videos of my child during activities for promotional purposes (e.g., website, social media). I understand that my child's name will not be used without further consent. Opting out means reasonable efforts will be made to exclude them, but accidental inclusion in group photos is not a violation.
